Target Audience – All Qualified Therapists, Nurse Associates, Staff Nurses, Ward Sisters, Community nurses, Clinical Nurse Specialists, Advanced clinical practitioners.

This course has been designed to explore the principles of Palliative and End of Life Care, as applied in all Trust care settings (acute & primary care hospitals and community settings). It aims to equip staff with the skills to complete an initial and ongoing holistic assessment of the needs of a patient requiring Palliative or End of Life Care. It will also explore and analyse the needs of the ‘family’ and the role of the multi-professional team in meeting these needs.

There is an expectation that all 3 days are attended in person, you should seek to secure managers approval prior to booking a place with completion of the managers declaration form at the back of the prospectus.

This session looks at factors that affect wound healing, holistic assessment and wound assessment.

The learning outcomes and content of the session as follows:

  • recognise changes in the individual's wound and surrounding skin and when to refer on for advice or support

Shared Decision Making 

Achieving Shared Decision Making goals of care is one the quality priorities for the trust .Shared Decision Making means people are supported to understand the care, treatment and support options available and the risks, benefits and consequences of those options, and to make a decision about a preferred course of action, based on evidence-based, good quality information and their personal preferences. 

It is a process in which practitioners and individuals work together to select tests, treatments, management or support packages, based on evidence and the individual’s informed preferences, rather than the individual being told what to do by the practitioner. 

Shared Decision Making is relevant in any nonlife threatening situation when a health or care decision needs to be made, and a range of options (including doing nothing), is available. 

This programme is approved by The Personalised Care Institute. 

Programme Structure 

A half day workshop, delivered face to face, designed to support practitioners to learn the process of good quality SDM and provide individuals with the knowledge and skills to support people in making decisions about their health and care. 

Our programme covers; 

  • Overview of Shared Decision Making
    • Brief introduction to how we make decisions
    • Brief introduction to health literacy Key elements of a shared decision-making conversation
    • Shared decision-making skills and practice 

This is a highly interactive programme with an emphasis on skills development and practice.

This training session will cover the essentials of skin integrity and pressure ulcer care.

 

The learning objectives and content of the session as follows:

  • identify risk factors associated with impaired skin integrity including: disease, surface, mobility, incontinence and nutrition
  • describe reasons why these factors contribute to risk
  • refer to appropriate members of the interprofessional team when required throughout the patient journey
  • identify appropriate interventions to reduce and manage risk of pressure ulcer development
  • implement appropriate prevention and management strategies

You have a maximum of three attempts to pass, you need a pass mark required of 80% or 8/10 to move on to the face-to-face training.

 

Step 3: Booking practical training

On completion of the e-learning training you MUST email over a screen shot of your pass mark to stees.clinicalskills@nhs.net to inform the team you have successfully completed the training by gaining a minimum of 80% or 8/10.

Following this email, please call the team on 01642 854062 to arrange your face-to-face practical training. At this point, the team will ensure they have received your pass mark via email.

Once attended, you will complete the designated competency pack within your clinical environment.

Target Audience – The sessions are aimed at senior nurses working at band 6 or above – who are completing or may need to complete DNACPR forms as part of their extended role.

Anyone who wishes to enrol for the DNACPR training must complete the Deciding Right’s e-learning prior to attending. A certificate will be issued on completion of the e-learning course. This training can be accessed via Northern Cancer Alliance .

Delegates must also have a written agreement from your line manager which outlines that the completion of a DNACPR form is appropriate to your role and is reviewed as part of the yearly SDR process.

The session will include communication in relation to DNACPR – principles are the same whether in community or the acute setting, legal considerations, practicalities of completing the DNACPR form – the same for community and acute setting and some case scenarios.
