Palliative Care Community Roadshow Education Programme is for Community Staff only.

Target Audience – All registered nurses and AHP's who deliver palliative and end of life care in the community as part of their work within the Trust.

This is a day 1 of a 2-day programme that provides a comprehensive overview of palliative and end-of-life care across in community care settings. Designed to build confidence and enhance the skills of staff working in the community, the programme focuses on supporting patients and their families with compassion and expertise.

Day 2: This course is linked with the Palliative Care Skills days (see page 7) which focuses on specific palliative care skills. You only need to attend the parts of day 2 you have not yet completed.

Key topics on day one include:

Recognising when a patient is approaching the end of life

Managing pain and other distressing symptoms

Communicating effectively and sensitively

Understanding and implementing advance care planning

The course is delivered by the Palliative and end of life clinical educators

Dr Trish Sealy is a consultant in palliative medicine working in the community and clinical lead for the Specialist Palliative Care team in South Tees. Trish will talk about when syringe drivers could be used, the medication used and provide guidance on how to source information on mixing medicines and compatibilities. This masterclass is suitable for all health care professionals who are involved in setting up, prescribing medication for use in syringe drivers and for those who are involved in caring for patients with syringe drivers.

e.g. delivering bad news, escalation of care, telephone v/s face to face conversation
• Knowledge and confidence in using SPIKES model
• Improve teamworking and patient safety through advanced communication skills

This training session will cover the essentials of skin integrity and pressure ulcer care.

 

The learning objectives and content of the session as follows:

  • identify risk factors associated with impaired skin integrity including: disease, surface, mobility, incontinence and nutrition
  • describe reasons why these factors contribute to risk
  • refer to appropriate members of the interprofessional team when required throughout the patient journey
  • identify appropriate interventions to reduce and manage risk of pressure ulcer development
  • implement appropriate prevention and management strategies
